wp-block-paragraph\">Le deuxi\u00e8me volet du CRT se consacre \u00e0 l\u2019accompagnement renforc\u00e9. Il s\u2019adresse aux personnes \u00e2g\u00e9es de plus de 60 ans, vivant sur le territoire d\u2019intervention du Centre et qui ont un GIR situ\u00e9 entre 1 et 4.<\/p>\n<\/div>\n<\/div>\n\n\n\n<div class=\"wp-block-columns alignwide has-ast-global-color-5-background-color has-background is-layout-flex wp-container-core-columns-is-layout-16cef306 wp-block-columns-is-layout-flex\" style=\"margin-bottom:0\">\n<div class=\"wp-block-column has-ast-global-color-3-background-color has-text-color has-background has-link-color wp-elements-d3435a3222ef861c6c35da22a9ddba47 is-layout-flow wp-block-column-is-layout-flow\" style=\"color:#000000;padding-top:2em;padding-right:2em;padding-bottom:2em;padding-left:2em\">\n<div class=\"wp-block-uagb-advanced-heading uagb-block-8e122caf\"><h3 class=\"uagb-heading-text\"><strong>Une prise en charge individualis\u00e9e<\/strong><\/h3><\/div>\n\n\n\n<p class=\"wp-block-paragraph\">Avant toute intervention, l\u2019\u00e9quipe pluridisciplinaire \u00e9valuera les besoins afin d\u2019identifier des objectifs d\u2019accompagnement et de soins. Ce document regroupera l\u2019ensemble des objectifs de travail et les actions men\u00e9es par le CRT et ses partenaires. Nous ne pourrons intervenir qu\u2019apr\u00e8s validation de la personne accompagn\u00e9e et de la commission d\u2019admission. Le projet initiale \u00e9voluera par la suite en fonction de l\u2019\u00e9volution de la situation et des besoins.<\/p>\n<\/div>\n\n\n\n<div class=\"wp-block-column
